Clarence Estate – Extended Aged Care – At Home (Dementia)
At Hall + Prior we tailor a range of programs that deliver the appropriate levels of care and service for all our clients, we recognize that for many people, the comfort and security of their own home is a more attractive option than moving into residential care. It is for this reason that we have developed special programs for people who want to live in familiar surroundings for as long as possible.
The Extended Aged Care At-Home (Dementia), or EACH-D packages, are tailored to meet individual needs and to help people diagnosed with dementia continue to enjoy the security of living in their own home.
Extended Aged Care At-Home - Dementia (EACH-D)
Extended Aged Care at Home Dementia (EACH-D) packages are individually planned and coordinated packages of care, they are tailored to help frail older people who have been assessed as experiencing behaviours of concern and psychological symptoms associated with dementia.
These behaviours and symptoms significantly impact upon their ability to live independently in the community and may also impact on their functional capacity to remain living at home. The EACH-D packages aim to assist older people diagnosed with dementia to remain living in their own home.
Who is eligible?
You must have been assessed by an Aged Care Assessment Team (ACAT) as needing high level care. The ACAT will also determine if you meet the criteria for an EACH-D package. On referral to the package, a Clarence Estate Case Coordinator will meet with you and any family members to plan what services may be appropriate for you.
Services available
The services that you may receive from the package will be individually tailored to your specific care needs. The types of services may include:
- Registered Nurse Care
- Specialist Dementia Assessment
- Care by an allied health professional such as physiotherapy, occupational therapy, etc
- Personal Care
- Home Help
- Social Support and after Hours Support Service
Who funds the programme?
EACH-D packages are funded by the Commonwealth Government. You will also be requested to pay a weekly fee as a contribution. For people on the maximum basic rate of pensions the fees will not exceed 17.5% of the maximum rate of the single pension. People with higher incomes may be asked to pay additional fees, which will be calculated on an individual basis.

Those experiencing different stages of dementia must always be treated with dignity and understanding. Clarence Estate staff has the required intelligence, compassion and the specialist training to be able to consistently provide this care.
