Clarence Estate Community Care Packages

Clarence Estate offers a unique community care program that services the Albany area. Four types of community care packages are available including Extended Aged Care at Home (EACH), EACH - Dementia (or EACH-D), Community Aged Care Packages and Transition Care Packages. An Aged Care Assessment Team (ACAT) assesses an individual's eligibility for community packages and can provide guidance on specific access to these packages. A Clarence Estate Community Coordinator provides further assistance to determine the specific services required.
