St LUKE's Aged Care Facility
St Luke's Aged Care Facility is a beautiful heritage listed homestead located in Subiaco, within walking distance to one of Perth's major landmarks, Kings Park. A bus stop is outside the front door and it is within walking distance to a few boutique shops. Accommodation at St Luke's includes single, double and multi-bed rooms with spacious activity areas and external gardens.
Admission to St Luke's Aged Care Facility is by approval of the Commonwealth Department of Health and Ageing, via your local Aged Care Assessment Team (ACAT). The Team will provide you with a copy of the approval, known as an ACCR Form. The Commonwealth Department of Health and Ageing also determines the fees and basic benefit paid for each resident. Information about the current level of fees may be obtained from the Director of Nursing at St Lukes.
CARE & SERVICES:
At St Luke's we employ Qualified Nursing Staff 24 hours per day 7 days a week, residents and their families have a choice of Medical Practitioner, we offer the services of Physiotherapy and can assist with Dental, Hearing, Optical and Podiatry upon request. We provide specialized nursing procedures as required and other services on request.
St Luke's has a Continuous Quality Improvement Programme in place, where upon all our services are under constant review to ensure that we always provide the highest standards possible. St Luke's accepts people from all religions and residents are encouraged to fulfill their particular religious needs, any Minister of Religion is free to conduct services at the facility.
MEALS:
Hall & Prior have developed an extremely skilled and highly experienced hospitality team including an Executive Chef; a Sous Chef; a Pastry Chef and a Dietician, to ensure that our residents are provided with a variety of nutritionally balanced meals. Choices of meals are available daily as well as special diets upon request. Daily menus are on display within the facility.

St Luke's offers an extensive activities programme designed to promote quality of life and entertainment for the residents which includes:
